  • Initial Consultation

    1 hr 20 min. Please call/email Fluid Chiropractic to book your 60 minute Initial Consultation: (720) 383-7536. We have allowed 80 minutes for you to fill out forms, process payments and book appointments. If you are more than 15 minutes late we will not finish the entire exam and your second visit will need to be extended to accommodate the full examination prior to treatment being given. Additional time used at the consultation is an additional fee. To minimize this please fill out the online intake form COMPLETELY. Areas commonly missed are past medical history. Fill out every section regardless of whether you feel it is pertinent to your current symptoms. This visit is Dr. Jennifer's opportunity to get to know you, your life and your body. This is a full history, full exam. The history is detailed and Dr. Jennifer is interested in everything. The exam includes ranges of motion, reflexes, strength testing, kinesiology testing, TMJ/cranial/abdominal/extremity exam and active/passive/control system tension pattern assessments. If you have had any recent lab tests, X-rays, MRI's please bring the reports with you, if possible. Please know the names of any medications/doses for your intake forms. Please have an idea of dates when you've had surgeries, accidents, emotional stresses and any details for your initial consultation. It is important to understand that Fluid Chiropractic does not work with any new members until they have been assessed by Dr. Jennifer and meet the necessary criteria for care.Dr. Jennifer looks forward to working with you and helping you to live your life the way you want to!
